Outpatients
The Family Medicine Center offers a Nicotine Dependence Clinic on Tuesday and Friday mornings for those interested in quitting tobacco use.
The Initial Assessment (pdf) (word) covers information such as
- frequency and longevity of tobacco use,
- past quit attempts,
- triggers and stresses that contribute to tobacco use, and
- a brief medical history.
Filling out and either emailing to ndp@med.unc.edu or bringing this assessment to the first appointment enables the treatment specialist to develop an individual's quit plan more quickly.
An individual treatment plan, developed during the first office visit, helps to guide the process of stopping all tobacco use. At follow-up visits, treatment plans for those who have stopped using tobacco and those who are still using tobacco will be based on the progress and barriers that the client has experienced during the preceding week.
